If you are interested in any of our services, begin by filling out the New Patient form below. If online forms are not your preference, feel free to print out the form (click here), complete it, and bring it into our office or fax it to us at 979.694.7337. Submitting online, via fax, or in person will take the same amount of time for us to process, but choose whatever feels most convenient for you.

We have psychologists and therapists that are in-network with most major insurance companies. We can often work with other insurance companies, and if insurance is not an option, we have sliding scale options available as well with our program at Oakwood Counseling. After you have submitted your form, we will verify benefits with your insurance company before calling to schedule an appointment with one of our therapists at Oakwood Counseling.
